

A recall for 81,893 2025 Lexus UX Hybrid, 2024 Lexus GX, Toyota Mirai, and 2024-2025 Toyota Land Cruiser Hybrid vehicles for an instrument panel that may fail to display certain warnings
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 Lexus UX Hybrid, 2024 Lexus GX, Toyota Mirai, and 2024-2025 Toyota Land Cruiser Hybrid vehicles. The instrument panel cluster combination meter may fail to display certain warnings or indicators. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 101, “Control and Displays” and in addition, for the subject Mirai vehicles, FMVSS 305, “Electric-powered vehicles: Electrolyte Spillage and Electrical Shock Protection.”

Descriptive Information
Although the involved vehicles are within the above production period range, not all vehicles in this range were sold in the U.S. This condition only affects vehicles that are equipped with a 12.3-inch combination meter containing specific software from a specific supplier. Other Toyota and Lexus vehicles are equipped with a combination meter that does not have the affected software. 100% of the involved vehicles contain a combination meter system with the software programming described in the Safety Recall Report. Whether this issue, in each case, will prevent the combination meter display from displaying certain indicators or warning messages and lead to the described safety risk depends on the conditions described in the Safety Recall Report, including whether a vehicle issue associated with those indicators or warning messages occurs while the vehicle is being driven.
Description of defect or noncompliance:
The subject vehicles are equipped with a combination meter located in the instrument panel that displays vehicle information, including certain indicators and warning messages, on a 12.3-inch display. The combination meter may experience a startup fault that can cause certain areas of the display to become blank and certain required indicators for engine oil pressure, engine coolant temperature, and electrical charge, may not be displayed when the corresponding underlying issue occurs. Therefore, the subject vehicles may not comply with FMVSS No. 101, paragraphs S5.5.3 and S5.5.4. In addition, for the subject Mirai vehicles, the blank display area may prevent the loss-of-isolation warning message from being displayed if that issue occurs. Therefore, the subject Mirai vehicles may not comply with FMVSS No. 305, paragraph S5.4.4. If these required indicators or warning messages are not displayed, the driver may not recognize the associated vehicle issue, which could delay driver taking the appropriate action and increase the risk of a crash or injury.

Chronology
In March 2024, Toyota began investigating this issue in Japan because it received customer complaint from the Japan market stating that certain areas of the meter display were blank after meter startup. While no abnormalities were found in the recovered part, Toyota conducted bench testing in May 2024 in an environment simulating meter startup. The phenomenon was reproduced but it was not clear if it was the same phenomenon the customer experienced. In June 2024, Toyota conducted a fault-tree analysis (FTA) and suspected that a malfunction could occur during meter boot due to the short time between boot processes and potential overlap.
In November 2024, the U.S. was added to this investigation because Toyota received a dealership product report from the U.S. market, which reported similar phenomenon to that observed in Japan. After further analysis, bench testing, and inability to produce the condition on an actual vehicle, Toyota developed a tool that could quickly cycle the combination meter on an actual vehicle and cycle tested repeating the startup sequence thousands of times to identify if the overlapping of processes could occur. This occurred between November 2025 to April 2026 and the testing was able to reproduce the condition. In May 2026, Toyota analyzed the testing results and determined that the overlapping of certain processes during the meter startup sequence could occur on an actual vehicle and cause certain portions of the combination meter to become blank.
Based on this information, on May 21, 2026, Toyota determined that the subject vehicles identified in Section 3 contain combination meter software that may cause the meter display to not display certain areas of the meter display during meter start up, and this could occur prior to first sale. If this occurs, the blank portions of the combination meter may cause the vehicle to not display certain indicators or a warning message even if an underlying vehicle issue associated with the indicators or warning is present. As a result, the subject vehicles may not meet the requirements of FMVSS No. 101, paragraph S5.5.3-4. In addition, the subject Mirai vehicles may not meet FMVSS No. 305, paragraph S5.4.4.

All known owners of the subject vehicles will be notified to return their vehicles to a Toyota or Lexus dealer. The dealers will update the software of the combination meter, free of charge.

Notifications to owners of the affected vehicles will occur by July 26, 2026. A copy of the draft owner notification will be submitted as soon as it is available.
